Novità regolatorie per allergie e influenza

L’Fda ha approvato l’estensione dell’indicazione richiesta per carbinossamina maleato (Karbinal ER di Tris Pharma) sospensione orale a rilascio prolungato indicata per le riniti allergiche, perenni o stagionali, ora anche nei bambini a partire dai 2 anni d’età in poi. Il medicinale è un antistaminico in formulazione orale, a lento rilascio del principio attivo che ne consente la somministrazione una sola volta al giorno, praticabile quindi anche nei bimbi e nei ragazzi che frequentano la scuola. Il farmaco è autorizzato anche per: rinite vasomotoria, congiuntivite allergica da cibi e inalazione di allerg eni, forme lievi e non complicate di reazioni allergiche cutanee come orticaria e angioedema; inoltre è una valida alternativa per tutti i casi di non responder agli antistaminici di seconda generazione che non sono soddisfatti della pesante posologia degli antistaminici di prima generazione. In Europa ma non dall’Agenzia europea dei medicinali, prima autorizzazione per il vaccino antinfluenzale quadrivalente di Gsk, in Germania e Regno Unito. Seguendo una procedura decentralizzata il Paul Ehrlich Institut Tedesco è stato il primo ente regolatorio europeo a concedere l’autorizzazione all’immissione in commercio al nuovo vaccino (Influsplit Tetra), seguito dalla Medicines and healthcare products regulatory agency (Mhra) agenzia regolatoria britannica che a sua volta ha autorizzato il farmaco (Fluarix Tetra). Per la prossima stagione influenzale 2013-2014 quindi il vaccino tetravalente, già autorizzato a fine 2012 anche dall’Fda (Fluarix quadrivalent), dovrebbe gi& agrave; essere disponibile in Germania e Gran Bretagna per l’immunizzazione attiva di adulti e bambini dai 3 anni d’età compiuti in poi. Il vaccino permette l’immunizzazione contro due sottotipi A del virus e due, invece di uno solo come nei vaccini in commercio finora, sottotipi B.(fonte farmacista33)

Ca colorettale: chirurgia laparoscopica o a cielo aperto?

La chirurgia laparoscopica come alternativa alla chirurgia a cielo aperto in pazienti con cancro rettale non ha ancora dimostrato di essere oncologicamente sicura. A tale proposito è stato progettato il COlorectal cancer Laparoscopic or Open Resection (COLOR II) trial, proprio allo scopo di confrontare la chirurgia laparoscopica e quella a cielo aperto in questo tipo di pazienti. Il trial si è svolto su 30 centri di 8 nazioni ed ha coinvolto 1.044 pazienti > 18 aa di età con Ca rettale senza evidenti metastasi a distanza. Lo studio è durato 6 anni. I pazienti sono stati assegnati, random in rapporto 2:1, alla chirurgia laparoscopica e alla chirurgia a cielo aperto, stratificandoli per centro, localizzazione del tumore e radioterapia preoperativa. I pazienti sottoposti a chirurgia laparoscopica hanno avuto una durata media dell’intervento più lunga (240 min vs 188; p> 0.0001) ma:
meno sanguinamenti (300 ml vs 400 ml; p< 0.0001)
più precoce ricanalizzazione intestinale (2 gg vs 3 gg; p< 0.0001)
più breve periodo di degenza (8 gg vs 9 gg; p=0.036).
Non ci sono state invece differenze significative per:
la compiutezza macroscopica della resezione
i margini delle resezione (p=0.250)
le complicazioni (40% vs 37%; p=0.424)
la mortalit&agrav e; a 1 mese (1% vs 2%; p=0.409).
In conclusione, in pazienti selezionati con Ca rettale, la chirurgia laparoscopica, eseguita da chirurghi qualificati, ha la stessa sicurezza, gli stessi margini di resezione, la stessa completezza della resezione di quella a cielo aperto. E in compenso si assiste ad un recupero migliore. I risultati dell’end-point primario – recidiva loco regionale – sono attesi per la fine del 2013. Berlusconi ruba la scena

Ci risiamo. Ancora una volta la parola conclusiva in quella che sempre più diventa la sceneggiata della politica ha un solo vincitore: Silvio Berlusconi. L’uomo che si fa sempre più amare e odiare ma con il quale prima o poi si deve avere a che fare ed è meglio per gli avversari non sottovalutarlo. Mostra d’essere non solo il padrone della Lega ma persino di una parte cospicua del Pd. Il suo merito è anche il demerito dei suoi avversari da Bersani che ha mostrato sia in campagna elettorale sia nel gestire il dopo elezioni tutti i suoi limiti e con lui, mi dispiace dirlo ma è così, lo stesso presidente della Repubblica.
Un Bersani che ha giocato tutta la sua partita alle lunghe trattative per la formazione del governo e ora ci si è messo lo stesso presidente della repubblica mentre il “paese brucia” e occorrono risposte immediate. L’errore di Bersani è stato quello di rendersi “incomprensibile” con la sua insistenza nel volere un’alleanza con il M5S pur sapendo sin dall’inizio che non sarebbe stata possibile. Alcuni osservatori politici ritengono che la mossa era tutta protesa alla ricerca di un alibi per ritornare sia pure con una formulazione diversa dalle “larghe intese” tra le braccia di Berlusconi “padre padrone”. Un gioco al massacro che, purtroppo, vede sconfitto lo stesso M5S dal quale molti avrebbero voluto non solo la protesta ma anche la spinta per il cambiamento. (Riccardo Alfonso fidest@gmail.com)

Italiani: un popolo ingovernabile?

Dalla sua unità sono passati 152 anni eppure tutta la sua storia e la cronaca di questi giorni ci danno una nazione divisa anche quando con la dittatura fascista si cercò una identità unitaria. Fu solo un fuoco di paglia che ci portò di filato in una guerra dalla quale uscimmo malconci ma non redenti. Diventammo forzatamente democristiani per ragioni di politica internazionale, ma con un partito comunista molto forte e temuto. Con lo sfaldamento della D.C. e dei suoi alleati, negli anni novanta, ci ritrovammo senza una guida ma alla mercé del primo imbonitore di turno. Così costruimmo un altro ventennio, dove demmo prova della nostra capacità di sopravvivere agli scandali, alla corruzione, al malgoverno, agli inciuci e alle panzane che ci ammannivano a manca e a destra. Ora ci ritroviamo con i cocci in mano, ma continuiamo a bramare il nostro messia politico e poco importa se ha dei difetti e delle debolezze. E’ lui che ci fa sognare il paese che non è e che non sarà a dispetto della cruda realtà. Per lui ci dividiamo, litighiamo, fanatizziamo e rendiamo sempre più confuso e triste la nostra vita. Non riusciamo ad avere la mente lucida, d’essere consapevoli che non è più il tempo di deleghe in bianco, ma che dobbiamo verificare di persona il mandato che noi affidiamo ai nostri eletti. Simoncini su offerta per Richard Ginori: tre i motivi di soddisfazione

Firenze – L’assessore regionale alle attività produttive e al lavoro, Gianfranco Simoncini, esprime apprezzamento per la presentazione del bando per la cessione della Richard Ginori. Lo ha detto al termine dell’incontro dell’incontro che ha avuto con il curatore fallimentare dell’azienda, Andrea Spignoli, che gli ha illustrato il bando che parte dalla proposta ricevuta da un gruppo che si è detto disponibile a pagare 13 milioni di euro e a riassorbire almeno 230 dei lavoratori.
“Mi sembra una prima buona notizia – ha spiegato l’assessore Simoncini – che induce ad un cauto ottimismo. Il primo motivo di soddisfazione è rappresentato dal riavvio della procedura che può portare alla vendita di un’azienda che rappresenta uno degli elementi portanti del manifatturiero toscano ed un simbolo della nostra regione. Il secondo deriva dalla certezza che oggi siamo di fronte ad almeno un soggetto disponibile a farsi carico di un rilancio di questo storico marchio. Il terzo è rappresentato dalla decisione del giudice di autorizzare l’esercizio provvisorio e di riattivare i forni, così da permettere di continuare a produrre”.“La Regione Toscana – ha concluso Gianfranco Simoncini – manifesta da subito tutta a sua disponibilità a sostenere tutti quei progetti che si presentino come credibili e che siano in grado di salvaguardare l’occupazione e la presenza di questa storica azienda a Sesto Fiorentino”.

Norway says ‘no way’ to pet reptile and amphibian keeping

Wildlife protection organisations across Europe are applauding a decision by the Norwegian government to retain a 37-year old ban on reptile and amphibian keeping and trading, and reject a proposal from wildlife dealers and exotic animal keepers to open trade in a limited number of species. European NGOs and wildlife professionals have supported the Norwegian Animal Protection Alliance in their campaign, over several years, to keep the ban.The announcement by the Norwegian Minister of Agriculture and Food was greatly welcomed by campaigners who have argued that lifting the ban would pose a threat to people and wildlife. A recent scientific study showed that at least 75% of reptiles die within one year in the home even though they have potential life spans ranging from 8-120 years, depending on species. As well as causing suffering to captive animals, the pet amphibian trade has been established as a route for the spread of pandemic wildlife disease that is threatening amphibian populations around the globe. Also, there are over 40 human diseases associated with reptiles and amphibians, which are of growing concern to public health experts.In 2009, several organisations commissioned an independent scientific assessment of the proposal to open trade, and the 65-page report concluded that under scientific evidence-based criteria, there are no amphibians or reptiles that make ‘suitable’ pets. Those campaigning to lift the ban argued that the illegal trade and keeping of these animals would be better addressed if the trade were legalised and monitored. That claim has been rejected. Says Anton Krag, biologist and CEO of the Norwegian Animal Protection Alliance: “We are delighted that our government has decided to give priority to animal welfare. The cruel smuggling and illegal keeping of wild animals should never be rewarded by legalising trade in these fascinating species.”Says Clifford Warwick, independent scientist and report author: “The Norwegian Government’s decision is fully in concert with the best, and rapidly increasing, scientific evidence which notes that the trade in reptiles and amphibians for pets involves serious detrimental threats and effects for global species and environmental conservation, indigenous wildlife, public health and animal welfare. The Minister’s finding emphasises the value of investment in of prevention over ‘cure’, which ought to be the objectively inescapable conclusion for any government faced with similar decision-making.”Daniel Turner, speaking on behalf of the Born Free Foundation, said; “The decision by the Norwegian Government must be applauded. Reptiles and amphibians should not be kept as ‘pets’ and the UK should follow Norway’s lead and humanely bring this form of wild animal exploitation to an end.”

World Bank Sees Climate Change Cutting Crop Production in Eastern Europe and Central Asia by over 25 Percent unless Action Is Taken Now

WASHINGTON In parts of Eastern Europe and Central Asia, climate change is poised to hamper food production and curb rural incomes over the next decades unless farmers get the help they need through improved water management and irrigation infrastructure, wider access to technology and information, and better land management and farming practices. Although governments throughout the region face rapidly narrowing windows of opportunity to protect farmers from climate change, there are actions that can be taken now to promote a new “climate-smart” approach to agriculture that improves agricultural productivity in today’s climate, while building resilience to climate change and reducing carbon emissions.Defining the policy and investment options available to governments – based on a rigorous evaluation of impacts of climate change on agricultural systems – is the aim of a new book published today by the World Bank, Looking Beyond the Horizon: How Climate Change Impacts and Adaptation Responses Will Reshape Agriculture in Eastern Europe and Central Asia.This new publication distills the experiences of four countries – Albania, Former Yugoslav Republic (FYR) Macedonia, Moldova, and Uzbekistan – that considered the future of their agricultural sectors under a number of climate change scenarios and examined each system’s capacity to cope with weather variability. Through economy-wide modeling of water supply and demand, the analysis found that, in many cases, water availability for irrigation will be severely curtailed by climate change. This will greatly exacerbate the effects of climate change on crops – especially irrigated crops – with potential yield reductions of 20–50 percent by 2050.The book also highlights the customized menu of climate change adaptation options devised by national experts and a World Bank team for each of the four countries. The adaptation options include practical measures such as improvement of crop varieties, investment in irrigation infrastructure, dissemination of timely and reliable weather forecasts to farmers, nutrient management and soil conservation, and livestock health and nutrition.The menus of adaptation options were prioritized for the various agricultural zones of all the countries according to rates of return on investment, acceptability to local farmers, applicability under different climate conditions, and the potential to reduce climate-change inducing greenhouse gas emissions. In several instances, the countries have already begun to put into action some of the recommendations. Within any economy, agriculture is one of the most sensitive sectors to climate change. In Albania, FYR Macedonia, Moldova, and Uzbekistan the risks are even more immediate and important given that the majority of the rural populations depend on agriculture for their livelihoods. The rural poor stand to be disproportionately affected by climate change because of this dependence, and because of their very limited access to the resources needed to adapt to shifting weather patterns. In the case of the four countries that participated in this program, all are projected to experience higher temperatures of 1.5 – 2.0 degrees C by 2050, and, on top of that, rainfall in Albania, FYR Macedonia, and Moldova is expected to decrease. More significantly for farmers in each of the four countries is that temperature increases and rainfall declines are expected to be greater in June to August, critical months for crop production.While the approach to analyzing climate change impacts, assessing adaptive capacity, and determining the costs and benefits of policy options and farm-level responses was tested in four countries in the Europe and Central Asia region, the three authors, Sutton, Srivastava, and James E. Neumann, emphasize that this model is sufficiently flexible – and yet necessarily rigorous – to be undertaken in any country.“We believe it is urgent and central to understand the scope of climate change, its impacts on agriculture, and the possible responses in this region. This study aims to address these concerns by building awareness about climate change in our client countries and to work with them to offer practical climate smart solutions,” said Dina Umali-Deininger, Agriculture and Rural Development Sector Manager in the World Bank’s Europe and Central Asia Region.In fact, in his recent speech at Georgetown University, World Bank Group President Jim Yong Kim called for increased investments in climate-smart agriculture. Complementing this call for climate-smart agriculture, the publication examines the scope for climate change impacts across a wide range of the most important crops in each agricultural area of the four countries and considers future production changes under three increasingly severe climate modeling scenarios. It also maps out clear, concrete adaptation responses to arrest production declines in each crop, from giving farmers access to better meteorological information through to improvements in drainage and irrigation.All four countries that partnered on this initiative have already begun undertaking some of the measures specified in the book, and the model is also now being applied to the agricultural sectors of Armenia, Azerbaijan, and Georgia.

Taste, Touch, Sight, Sound and Smell

After the launch at last year’s Milan Design Week, BE OPEN returns this year at Università Statale as co-producer of INTERNI magazine’s Hybrid Architecture & Design exhibition, from 8-21 April, with an installation by Christophe Pillet in the University’s Cortile della Farmacia, the BE OPEN Talk, a panel discussion moderated by Philippe Daverio on the sensory future and the BE OPEN Food Theatre at the Moroso showroom, a unique food experience. These events are a part of an extensive programme to research design for the five senses through innovative projects that reach out to the sixth sense, or intuition. The House of the Senses is a contemplative pavilion located in the center of Cortile Farmacia and integrated with the existing geometry of the formal garden. The three walls of the pavilion are screens for moving images created by Studio Azzurro. Compression Ignition is Emerging as a Key Technology in European Natural Gas Commercial Vehicle Market, finds Frost & Sullivan

London Europe is set to emerge as a strong regional market for medium- to heavy-duty natural gas (NG) commercial vehicles, populated by several competitors on both OEM and supplier sides. Driven by energy price volatility, tightening emission norms, and the shale gas revolution in the US, the market for NG vehicles is gaining considerable momentum. Of all alternative NG technologies, compressed natural gas (CNG) and biomethane technologies pose the least pressure on existing infrastructure. By 2018, the NG market in Europe is expected to reach production levels of nearly 18,000 units. Spark ignited technology will account for around 90 per cent of commercially manufactured NG buses, while compressed ignition technology will dominate the liquefied natural gas (LNG) truck market with around 60 per cent share. The heavy duty segment is set to account for 75 per cent of NG truck sales with LNG being a dominant fuel option. OEMs’ willingness to differentiate products through technology partnerships is leading toward increasing focus on compression ignition and dual fuel technologies. Compression ignition would facilitate the use of NG vehicles for long haul application, which would compensate for the higher upfront cost – provided the fuel infrastructure and diesel NG price differential exists. In future, the margins for module suppliers will shrink, and the same will happen for component suppliers when OEMs begin exerting pricing pressures as volumes start to grow. Duty cycle restriction of NG vehicles can be overcome through concerted strategies aimed at developing vehicles and products that deliver highest efficiencies in certain targeted vocations and duty cycles along with necessary fuel infrastructure. Il M5S non è un “valore aggiunto”

Non lo è per gli altri partiti nel momento in cui trattano i loro componenti, presenti nei due rami del Parlamento, come una loro creatura, nonostante ritengano sia stata “estorta” col voto degli italiani. Da qui emerge, ad esempio, il “buonismo” di Bersani nell’incontro pubblico con i capi gruppo della Camera del M5S. Come dire: “ragazzi” lo so che siete una nostra costola e allora comportatevi di conseguenza invece di “giocare” al bastian contrario. Si sbaglia Bersani, ma non è il solo. Anche in casa Pdl la pensano allo stesso modo.
Il M5S è indubbiamente l’espressione di un trasversalismo conseguente alla cattiva gestione della politica negli affari di Stato ma non può essere trasformato in un’operazione aritmetica da parte di chi per anni ha sguazzato nel torbido. E’ vero, tuttavia, che al sistema va tolta l’ingessatura e si potrebbe fare anche un’eccezione favorendo l’avvio del governo a guida Pd ma restando poi alla finestra a verificare se continua a ciurlare nel manico come era solito in passato. Come dire: mettiamoli alla prova e vediamo se sono capaci di saper fare ciò che affermano ma si devono impegnare ad impiegare un loro percorso virtuoso in tempi rigidamente prefissati, pena la sfiducia. Questo mi pare un compromesso onorevole e comprensibile anche da parte di chi ha fatto della protesta e della diffidenza il suo naturale cavallo di battaglia. (Riccardo Alfonso fidest@gmail.com)

Politica: la logica di appartenenza

Un altro tema caro ai politici che militano nei partiti è di voler riconoscere nei parlamentari del M5S qualcosa di “familiare”. Taluni dicono, infatti, che hanno una mentalità di “centro destra” e altri no essendo per lo più di “centro sinistra” e così dicendo si consolano pensando di poterli asservire, prima o poi, alla loro causa. A questo punto è forse noioso ripetere le stesse cose ma, a volte, è necessario, anche se potrebbe diventare un atto disperato se pensiamo che non ci sia peggiore sordo di chi non vuol sentire. Per anni Grillo ha cavalcato la protesta di quanti sono stati bellamente presi in giro dal nuovo che si prospettava e che diventava regolarmente il vecchio che si perpetuava con le mummie che li rappresentavano. Il cambiamento a questo riguardo non s’identifica più con la logica delle alleanze tra partiti, ma sulle cose da fare, e fare non significa solo dire, ovviamente. Ecco perché in casa Pd dovrebbe prevalere il convincimento che se il programma del M5S è affine al loro ciò che quest’ultimi possono garantire al Paese è un qualcosa che il PD e lo stesso PDL non possono più dare: la fiducia. E allora la governabilità esiste e con essa le maggioranze parlamentari qualificate. Si tratta solo di capire il nuovo che si presenta e di saper cogliere il diverso che non significa avere un partito padronale, da una parte, e le confuse anime sull’altra sponda, ma una cultura del cambiamento. Sungrow’s SG30KTL Obtains Double-A Grade from Photon Lab’s Test

Munich, PRNewswire Sungrow’s SG30KTL has been awarded a double-A grade evaluation (“Sehr Gut”) by Photon Lab for both medium and high irradiation, commendably following its siblings, Sungrow’s 4kW and 15kW inverters. Governo: facciamo il punto della situazione

Le recenti elezioni politiche ci hanno dato un esito in pratica “interlocutorio” nel senso che non ha sancito un vincitore ma, semmai, tre mezze vittorie, mentre il partito di Monti si è fermato al 10%. La logica avrebbe voluto che si sarebbe potuto uscire dall’attuale impasse se due dei tre partiti si fosse alleato. Lo ha chiesto il Pdl pensando al Pd ma quest’ultimo lo ha chiesto al M5S ritenendolo più affine. Ora con il rifiuto del M5S d’unire le sue forze con il Pd non per motivi d’incompatibilità di programmi ma per ragioni di fiducia la situazione si è complicata. Taluni osservatori politici, per lo più adusi a ragionare con la logica dei vecchi schemi, intravedono, in questa mossa del M5S, un “irragionevole irrigidimento.” In questo modo di esprimere la loro contrarietà si potrebbe persino percepire una sorta di malafede poiché non si può pensare che hanno ignorato la ragione di fondo che ha animato da anni il movimento di Grillo e condiviso oggi da milioni d’italiani. E’ il frutto, semmai, che sta andando a maturazione di un disagio crescente di quanti non riescono più a identificarsi con la politica che danni parla di riforme e regolarmente le disattende. Partiti che hanno avuto bisogno di un governo extraparlamentare per uscire dalla profonda crisi di sistema e che ancora oggi annaspano nel buio assoluto. Basterebbe che il Pd lo riconoscesse pubblicamente e affidasse al M5S il governo del Paese offrendogli la fiducia. Errare umanum est ma “perseverare” est diabolicum.
