IQVIA Collaborates with Cancer Researchers to Advance Use of Real World Evidence

IQVIA™ (NYSE:IQV) is proud to announce its collaboration with Friends of Cancer Research in a cross-industry effort to advance acceptance and drive future uses of real world evidence (RWE). This research helps confirm that clinical benefits seen for therapies in clinical trials were consistent with the benefits of those treatments within real world settings. RWE comprises dynamic data including assets such as EMR, hospital, pharmacy, and claims sources, as well as genomics, mobile health, and patient reported outcomes. The project is part of a Friends of Cancer Research initiative that is working to demonstrate the application of real world evidence in oncology research. As part of this project, a recent study involving six research centers in the U.S. followed a common protocol to assess real world endpoints among cancer patients, including overall survival, time to next treatment, time to treatment discontinuation, time to progression and progression-free survival. Results from the collaborative project, An Exploratory Analysis of Real-World End Points for Assessing Outcomes Among Immunotherapy-Treated Patients With Advanced Non–Small-Cell Lung Cancer, were recently published in JCO Clinical Cancer Informatics. Researchers used non-identified patient data from assets such as administrative claims and electronic health records to assess real world endpoints and found that they were generally consistent with each other and with outcomes observed in randomized clinical trials.

Study adds to evidence that electronic cigarettes are not harmless

A study published in JAMA Cardiology has added to growing evidence that electronic cigarettes (e-cigarettes) are not harmless. “Studies like this give further confirmation that e-cigarettes are not harmless,” said European Society of Cardiology cardiovascular prevention spokesperson Professor Joep Perk.“If I was a minister of health I would put my efforts into public anti-smoking campaigns especially directed towards the younger generation, and not promote e-cigarettes as an alternative to smoking,” he continued. “There are studies also showing that people that start with e-cigarettes have a tendency to become persistent tobacco cigarette smokers as well.” The 2016 European guidelines on cardiovascular disease prevention flagged up the need for further research on the long-term effects of e-cigarettes. The current study included 23 habitual e-cigarette users (used most days for at least one year) and 19 non-users between the ages of 21 and 45 years. It found that habitual e-cigarette users were more likely than non-users to have increased cardiac sympathetic activity (increased adrenaline levels in the heart) and increased oxidative stress – known mechanisms by which tobacco cigarettes increase cardiovascular risk. The authors said the findings “have critical implications for the long-term cardiac risks associated with habitual e-cigarette use” and “mandate a re-examination of aerosolized nicotine and its metabolites”. They added that causality could not be confirmed on the basis of this single, small study, and that further research into the potential adverse cardiovascular health effects of e-cigarettes is warranted. “Nicotine stimulates the central nervous system, so it’s not at all surprising that people continuously taking nicotine get this sympathetic stimulation,” said Professor Perk. “This then might lead to irregular heartbeat and raised blood pressure, and probably has long-term deleterious effects on the blood vessel walls.” “It is too large a step to say that these negative effects are proof that people are going to die early because they used e-cigarettes,” he continued. “To prove this you have to put people on e-cigarettes for 10 to 15 years and see how many die early – a study that will not be done for ethical reasons. The weakness of all studies in this field is that they are observational and small, and they look at indicators of vascular wall damage rather than incidence of cardiovascular disease or death.” Professor Perk said that, even after this study, e-cigarettes could still be used to help people stop smoking tobacco cigarettes, but they should be used with caution and other methods should preferably be tried first. He said: “E-cigarettes are one of the tools we have in nicotine replacement therapy but as clinicians we should be cautious of putting people on large amounts of central nervous system stimulant drugs. Other smoking cessation schemes, such as chewing gum or patches, always include the decision to taper off use and eventually stop. This is not in general the case with e-cigarettes, which tend to be seen as a replacement and not a weaning off nicotine addiction. In fact they prolong the addiction.” “This is an area where we need more knowledge,” continued Professor Perk. “The more data we collect, the more it seems that nicotine replacement strategies that taper off and ultimately end nicotine use are the way to go.” “At the end of the day the best thing is simply to prevent people ever getting into the vicinity of nicotine,” he concluded.

“European Evidence Project”

Project aims at providing a road map (guidelines, recommendations, technical standards) for realising the missing Common European Framework for the systematic and uniform application of new technologies in the collection, use and exchange of evidence. This road map incorporating standardized solutions would enable policy maker to realize an efficient regulation, treatment and exchange of digital evidence, LEAs as well as judges/magistrates and prosecutors and lawyers practising in the criminal field to have at their disposal as legal/technological background a Common European Framework allowing them to gather, use and exchange digital evidences according to common standards and rules. I was called to be part of the EVIDENCE Project as the External Advisory Board component. The project was structured in modules that have been developed by working groups of different nationalities. Among other topics I intend to focus on the terms of usability and validity of digital evidence in various legal-state of the European Community, as specified below.
1) Learn from practice extrapolating rules on which to base the communication protocols;
2) Verify the differences between those rules and the rules of the member states;
3) Identify suggestions to be submitted to European level for a possible elaboration of specific recommendations to the individual states;
4) Distinguish into the various meanings of the word “evidence”;
5) Develop a comprehensive definition of “electronic evidence”;
6) Realize tables or maps of comparison and connection between the elements belonging to the same level of the structure in the various states, to make possible to know in advance with a reasonable degree of certainty the level of usability of the electronic evidence that is transmitted from an authority of the State A to an authority of the State B, and consequentially to modify or integrate the electronic document in view of this result.
Conclusively electronic evidence must be configured as an object, with its own taxonomy by reason of the moment investigative or procedural in the various jurisdictions and the representation of the related possibility of use at each of them.

White House Sees Value of Ag Export Programs

Amid the serious cuts the White House has proposed for key agricultural programs, there are a few bright spots. The U.S. Grains Council applauds the administration for recognizing the value of expanded agricultural exports in its proposal for mandatory spending cuts. “It’s recognition for the benefits agricultural exports bring back to the U.S. economy,” said Floyd Gaibler, USGC director of trade policy. “It’s evidence that the work of joint government-industry groups like the Council is paying off for American farmers, for American business, and for our nation’s economy.” The administration recommended extensive cuts in USDA spending to the so-called “super committee” working on a government-wide deficit reduction plan but specifically cited “increased funding for programs to expand U.S. agricultural exports” as one of the ways it supports U.S. agriculture. Other examples of programs it supports included the pursuit of trade agreements, agricultural research programs, and assistance for beginning and disadvantaged farmers. In contrast, the White House recommended eliminating direct payments, reducing subsidies to crop insurance companies (including a change in subsidies for farmers’ crop insurance premiums), and reducing conservation spending by targeting it to the most environmentally beneficial programs.

Mental health conditions

The WHO report demonstrates that people with mental health conditions are vulnerable – not because of any inherent weakness, but as a result of the way they are treated by society.  It illustrates how people are not only missed by development programmes, but can be actively excluded from these programmes, this despite the fact that an explicit goal of development is to reach the most vulnerable. It also presents a number of evidence-based strategies which are known to improve development outcomes for persons with mental health conditions and all vulnerable groups. The WHO report is a call to action to all development stakeholders – multilateral agencies, bilateral agencies, global partnerships, private foundations, academic and research institutions, governments and civil society – to focus their attention on mental health. Overweight women

Rome, Italy: Being overweight leads to a greater risk of miscarriage for patients undergoing assisted reproductive technology (ART), the 26th annual conference of the European Society of Human Reproduction and Embryology heard today (Monday). Dr. Vivian Rittenberg, a Clinical Fellow in the Assisted Conception Unit, Guy’s and St. Thomas’ Hospital NHS Foundation Trust, London, UK, said that her research provided additional evidence to show that increased body mass index (BMI) was independently associated with a higher miscarriage rate after IVF or ICSI treatment. This information should be included in the counselling given to patients before they undertake ART, she said. Overall, 26% of women miscarried before 20 weeks gestation. The researchers found that the miscarriage rate was significantly lower in women with normal weight (22%) compared to women who were overweight (33%). After adjusting for other variables that might have skewed the results such as age, duration of infertility, smoking and a history of previous miscarriage, they were able to show that being overweight or obese more than doubled the risk of miscarriage. “Overweight women wishing to get pregnant by spontaneous conception are already counselled to lose weight before trying for a baby,” said Dr. Tarek El-Toukhy, Consultant in Reproductive Medicine at the Assisted Conception Unit, Guy’s and St. Thomas’ Hospital NHS Foundation Trust, London, UK, who led the study. “Our findings have shown clearly that women undertaking ART should be strongly encouraged to heed this advice in order that they can have the best possible chance of obtaining and maintaining a pregnancy.”

Diabete: dalla cura al prendersi cura

Roma 25 maggio, ore 10-13 Piazza della Minerva 38,  Biblioteca del Senato “Giovanni Spadolini” – Sala degli Atti Parlamentari Subito! il grande progetto 2009-2013 della diabetologia italiana – “dalla ‘cura’ al ‘prendersi cura’ della persona” Negli ultimi anni il messaggio forte emerso dall’evidence based medicine nella cura del diabete è stato che il trattamento intensivo e precoce, attuato con immediatezza alla diagnosi, determina una prognosi più favorevole, con minor rischio di complicanze. La strada giusta non è far scendere la glicemia quando si è già assestata su valori scadenti, ma intervenire tempestivamente alla diagnosi o alla presa in carico, per raggiungere nei primi mesi il controllo ottimale della malattia. Per questo nasce SUBITO!, il progetto concepito da AMD – Associazione Medici Diabetologi, che si pone come obiettivo principale di migliorare il compenso metabolico della persona con diabete, cioè riportare i valori della glicemia alla normalità e mantenerli costantemente sotto controllo, sin dall’esordio della malattia o comunque alla sua diagnosi, al fine di ridurre il peso delle complicanze cardiovascolari nei successivi 5 anni.
