9th Congress of the Asia Pacific Initiative on Reproduction

More than 1,700 fertility specialists from 50 countries are attending the 9th Congress of the Asia Pacific Initiative on Reproduction (ASPIRE 2019) in Hong Kong to explore the latest advances in scientific research and clinical care of people experiencing infertility.It is one of the most influential meetings on assisted reproduction ever presented in the Asia Pacific region that is home to about 60 per cent of the world’s population.ASPIRE President, Professor Budi Wiweko, a leader in the advancement of IVF and associated technologies in the region, said outcomes from the congress would empower the region and help shape the future of assisted conception throughout the world.
People attending the congress at the Hong Kong Convention and Exhibition Centre include scientists, clinicians, embryologists, nurses and fertility counsellors to discuss and dissect future aspects of reproductive medicine.One in six couples throughout the world experience infertility, which is defined as the failure to conceive after a year of unprotected intercourse, or the inability to carry pregnancies to a live birth. The causes of infertility are equally shared between male and female partners.The World Health Organisation has forecast that infertility will become the third most serious condition after cancer and cardiovascular diseases in the 21st century.
Delegates at the ASPIRE 2019 Congress have come from across the Asia Pacific, Europe, the United Kingdom, Middle East, India, Pakistan, Australia, New Zealand, North America, South America and Canada.Professor Wiweko said ASPIRE’s widening scientific exchange with leaders in reproductive medicine in other countries would be vital in addressing a disturbing decline in fertility rates across the world, and especially in the Asia Pacific region.
There are many complex factors contributing to this trend including couples marrying later in life, growing urbanisation and associated stress levels, work pressures and environmental conditions.

Major Initiative to Close Information Gaps on Cross Border Reproduction in the Asia Pacific

A powerful task force of scientists and clinicians specialising in assisted conception have mounted a major campaign to close gaps in knowledge about cross border reproduction in the Asia Pacific region.Each year, thousands of infertile couples travel the world for third party reproduction in the quest to achieve their dream of parenthood.The Asia Pacific region, in particular, has been a booming market for procedures such as sperm, egg and embryo donation and gestational carriage, or surrogacy.However, the extent of this activity is not fully understood and there are concerns about the quality and safety for all stakeholders, including children born from these assisted reproductive procedures.The 9th Congress of the Asia Pacific Initiative on Reproduction (ASPIRE 2019) being held in Hong Kong will progress an initiative to gather vital information on cross border reproductive care from 21 countries in the region, and implement a framework of consistent guidelines and standards of care.Dr. Clare Boothroyd, one of Australia’s leading fertility specialists and Vice-President of ASPIRE, said the survey would be far reaching to engage with donors, recipients, brokers, providers of infertility services, governments and community groups.Many couples seeking cross border reproduction through treatments such as oocyte donation or gestational carriage have to travel across a minefield of varying legal and regulatory controls in different destinations.Unmarried couples experiencing infertility, and same sex couples wanting to have a baby, face even more challenging barriers to treatment in many countries.“Many people throughout the world are willing to travel to access assisted reproductive procedures that may be prohibited, restricted or too costly in their own countries,” Dr. Boothroyd said.

Medically assisted reproduction

The European Society of Human Reproduction and Embryology (ESHRE) has published today (Thursday) a position paper related to intrafamilial medically assisted reproduction (IMAR).This particular type of assisted reproduction can raise various ethical and controversial issues, due to the involvement of a family member as a third party.The ESHRE Task Force on Ethics and Law acknowledges the benefits that IMAR may bring to those choosing this approach and concludes that certain forms of IMAR are morally acceptable under certain conditions. The group advises to evaluate each request for IMAR individually, based on four ethical principles in health care: the respect for autonomy, beneficence and non-maleficence and justice.

Potential risks may affect several parties, including the future child. These risks can arise from intrafamilial conflict if parents feel threatened in their parental role or if they have different views from the collaborators on how the child should be informed of its genetic origins. Especially in cases of intergenerational IMAR, there are concerns that the child may be confused about his role in the family. The possible pressure on the donor or surrogate to collaborate can also lead to psychological problems. Adequate information on possible risks should be given to all parties. This includes both combined and separate counseling of recipients and collaborators to assess the voluntariness of the donation and to reduce potential conflict situations.
Part of adequate genetic counseling and good clinical practice in such cases is to offer carrier screening for those disorders that are more prevalent in the particular ethnic group. Given that fertility specialists have a co-responsibility for the welfare of the child, it may be morally justified to offer such genetic testing as a condition for access to assisted reproduction.  The group concludes that in some situations IMAR is morally acceptable as long as counseling of recipients and collaborators is applied in order to reduce potential psychosocial and medical risks. First-degree intergenerational IMAR needs special scrutiny, also in view of the increased risk of undermining autonomous choice. First- and second degree consanguineous IMAR is at odds with the spirit of anti-consanguinity and anti-incest legislation in most countries and should not be offered. The group encourages more research into the psychosocial implications of IMAR to contribute to adequate and moral guidance. Meeting Society of Human Reproduction and Embryology

The European Society of Human Reproduction and Embryology (ESHRE) announced today that it has chosen Istanbul for its 2012 Annual Meeting. During 1 to 4 July 2012, ESHRE will hold its Annual Meeting at the Istanbul Congress Centre (ICC).  Reproductive medicine is one of today’s most exciting scientific and clinical areas. Every month there are announcements of medical ‘firsts’, new rulings by regulatory bodies or ethical controversies. Children: Medically assisted reproduction

Children conceived by Medically Assisted Reproduction (MAR) have fiscal implications for government both in terms of future government spending and tax revenue. Based on public funding to conceive a MAR child – after factoring in education, future health and pension costs, and future tax contributions of this child – the discounted net tax revenue (the difference between future government spending and tax revenue) of a child born in 2005 is roughly €127,000 in today’s value. Considering an average treatment cost of approximately €15,000 to conceive an IVF-child, this represents an 8-fold return on investment (ROI) for governments. While the costs of MAR treatment represent a substantial proportion of a patient’s annual disposable income, MAR typically represents less than 0.25% of total national healthcare expenditure. By comparison, obesity accounts for 10% and 2-4% of total health care spending in the US and Europe respectively. MAR treatments elicit significant medical, reproductive and economic influence in developed countries with 3.5 million children estimated to have been born worldwide since 1978. These children make up a substantial proportion of national births with up to 4.1% in Denmark and 3.3% in Belgium. In the US, Europe, and Oceania over 600,000 treatment cycles resulted in 120,000 children being born in 2005. The European Society of Human Reproduction and Embryology (ESHRE) Task Force on ‘Reproduction and Society’ reviewed the economics of MAR to evaluate the benefits of funding of MAR for society and to inform policy makers on effective, safe and equitable financing of MAR. Dr. Mark Connolly and colleagues who published this review paper in the journal Human Reproduction Update based their findings on key epidemiological and economic studies. The authors conclude that the way MAR is subsidised in different healthcare settings and for different patient groups has far-reaching consequences for access to treatment, clinical practice, and infant outcomes. MAR children deserve the best start in life, and as discussed in this review, affordable treatment can make economic sense as well.ESHRE was founded in 1985 with a mission is to promote the understanding of reproductive science and medicine. Reproduction under new savings scheme

The Danish government until now provided reimbursement for assisted reproduction treatments (ART) with up to three treatment trials for married and unmarried couples, singles and homosexuals. In light of the need to prioritise public expenditure, the Parties agreed this month to implement a legislative amendment, so that Assisted Reproduction Treatment (ART) will no longer be part of the free public health services. ‘This is sad news considering Denmark has always been praised as the benchmark in Europe in terms of access and availability of treatment for patients seeking ART’ said Dr. Søren Ziebe from the University Hospital in Copenhagen, Executive Committee Member of ESHRE and the SIG coordinator of all SIGs at ESHRE. The Danish government has allocated a pool of a mere 50 million Danish Krone (approximately 6.7 million Euros) for groups with special needs only. The cuts in this area will decrease government spending by 200 million Danish Krone (EUR 26.9 million) annually in 2011, 2012, 2013 and thereafter. In 2007, 4.9% of all children born in Denmark were babies conceived with the help of ART, according to the ESHRE European IVF Monitoring Group (EIM). With 2,558 cycles per million inhabitants annually, Denmark has the highest availability of ART in Europe. ‘Every school class in Denmark has two IVF children on average’ said Dr. Ziebe. ‘These changes will have major implications and detrimental effect for childless couples, for fertility clinics and the research environment in Denmark.’:http://www.eshre.eu/page.aspx/1020

Nevertheless language is also in a perpetual state of flux and the number of women who play a part in determining the language that we speak is increasing. The number is growing just as women’s bodies in general have been designed for expansion and reproduction. Still in 2009, impressed with towers, the money machine is falling apart unfortunately but reproduction remains. We need to talk again. We have to talk about how we solve this problem with the MONEY. No more quick promises, we want the realities. Invest more in real estate, but then the endless new properties stand empty, therefore we need paintings to fill these properties with meaning. Or refugees. Migrants are always grateful for the large number of empty real estate speculation projects. How much space does a person need and can he pay for it? Can he pay for the image of a person. So you pay me for my picture. No, this sort of thing doesn’t actually belong in a press release, I just wanted to seduce you to buy one of my works. With my charm, my evening gown and my biography, I wanted to seduce you into buying a painting from me.
